If you play through the entire game and defeat the final boss Salvatore, the game will restart in Hell mode at Agran Village. You will still have all of your original skills and equipment as of the time you defeated Salvatore, however the weakest enemies in existence in the game are now at level The available quest set is now different, there are a handful of new one-time quests and multiple new repeatable ones in the various locations.
